Program Definitions

Intensive Out Patient (IOP) – CD, MICD or Complex Co-Occurring

Clients in IOP programming usually stay in housing provided through Park Avenue Center.  While not required, it is encouraged.  While in IOP, clients attend programming 7 days a week.  Clients attend programming between the below hours, varying based on the day of the week.

Men’s: 8:00am to 5:00pm M-F, Weekend mornings
Women’s:  8:00am to 8:00pm M-F, Weekend mornings

 

Out Patient (OP) – CD or MICD

Clients in OP programming are not provided housing.  Actual schedule is determined between client and primary counselor in the treatment plan.  Available scheduling hours:

Men’s:  Daytime Outpatient approximately 8:00am to 4:30pm
Men’s:  Evening Outpatient approximately 6:00pm to 9:30pm
Women’s:  Daytime Outpatient approximately 8:00am to 4:30pm, 4 days/week

 

Relapse – CD, MICD or Complex, OP

Clients who have previous completed treatment but have experienced recent, short-term relapse can attend Out Patient programming to get back on track with their recovery.  Actual schedule of treatment is determined between client and counselor in the treatment plan.  Available scheduling hours:

Men’s:  Daytime approximately 8:00am to 4:30pm
Men’s:   Evening approximately 6:00pm to 9:30pm
Women’s:  Daytime approximately 8:00am to 4:30pm

 

African-American Specific – CD or MICD, OP or IOP

This program modifier can be added to CD or MICD care levels.  This program track allows the ability to talk about race and the issues and pain that have arisen, empowering clients to overcome their sense of victimization by acknowledging the historical context and developing new behavioral methods to cope.  Available scheduling hours follows above OP and IOP schedules.

Park Avenue Center can not accept referrals for adolescent clients, or clients who have a sex crime conviction.

SCHEDULING INTAKES

Please call 612-871-7443 Monday through Friday 7am to 4pm and speak with our Admissions Team to schedule an admit. Referrals that specify the care level and have the funding in place are usually processed within 48 hours. Once the referral has been processed, admissions will contact the client to schedule. Providing good client contact numbers and a client email if available helps ensure that the client can be reached. Admissions can also coordinate transportation from the following facilities:

  • AIDC 1800 Detox transports are done Monday through Friday.
  • Hennepin County jail pick-ups are done Monday thru Friday.
  • Hennepin County workhouse pick-ups can be scheduled Monday thru Friday.

The above transports can be arranged when the admission is scheduled.

HOUSING

Housing at Park Avenue Center is a combination of duplexes and small unit apartment buildings located in south Minneapolis. The residences are warm, inviting and meticulously maintained.  Each residence has a washer and dryer that is free for client use. Kitchens are fully stocked with cooking equipment and basic staples. Clients are taken grocery shopping each week to select their own food and the food is paid for by Park Avenue Center. Clients living in the housing are transported to and from the treatment buildings. House managers are present nightly. Room and board costs are absorbed by the facility.
